Number of Office Visits: Per person per policy year

The policy year is a 12 month period that begins from the insured's effective date and ends 12 months later.

Example: Mr. Smith a member decides to purchase the Diamond health plan with an effective date of July 1, 2009. Mr. Smith's policy year is July 1, 2009 through June 30, 2010.
